Request Certificates of Compliance or Copies of Financials

Requests for Copies of Financial Statements

Requests for copies of financial statements must be submitted to the Department in writing.

The Department charges $.50 per page for copies. In order for copies to be mailed to the requestor, the Department must be provided with the requestor’s account number with a courier service, as well as an address and contact person to which an invoice can be sent.

There is no cost for Certificates of Compliance or Certificates of Similarity.
